INVITATION FOR EXPRESSION OF INTEREST AND INVITATION TO TENDER

1. INTRODUCTION
The University of Nigeria Teaching Hospital (UNTH), Enugu is one of the first-generation tertiary hospitals designated as Cardiothoracic Centre of Excellence in Nigeria, which provides specialized medical services to the whole southeast region and beyond.

Therefore, in line with BPP’s guidelines and provision of the Public Procurement Act 2007, the Management of University of Nigeria Teaching Hospital, Ituku/Ozalla, Enugu hereby invites credible, competent and experienced bidders/contractors and service providers to submit their technical and financial bids for the under-listed projects:

LOT 1S: PROVISION OF MEDICAL OUTREACH IN IGESHA TEDO ITIRE, ORILE, COKER, IKATE, AGUDA CONSTITUENCY
LOT 2S: MEDICAL OUTREACH IN SELECTED COMMUNITIES IN ENUGU WEST SENATORIAL DISTRICT
LOT 3S: PROVISION OF MEDICAL OUTREACH IN SURULERE COMMUNITY
LOT 4S: EVALUATION AND VALIDATION OF ALL THE ASSETS OF UNTH ITUKU OZILLA AND THE OUTPOSTS.

3. ELIGIBILITY REQUIREMENT
Interested and competent contractors, supplier and service providers/consultants wishing to carry out any of the projects listed above are required to submit the following documents in their technical bids/proposal.
a. Evidence of certificate of incorporation issued by the Corporate Affairs Commission (CAC) including, form CAC 1.1 or CAC 2 and CAC 7. Business name with BN1 is also acceptable for consultancy Services
b. Evidence of Company’s Income Tax Clearance Certificate (or personal income tax clearance certificates of all the partners in case of business name) for the last three (3) years valid till 31st December 2024;
c. Evidence of Pension Clearance Certificate valid till 31st December 2024 (this requirement is only applicable to bidders whose number of staff is 15 and above);
d. Evidence of Industrial Training Fund (ITF) compliance certificate valid till 31st December 2024 (this requirement is only applicable to bidder whose number of staff is 5 and above or bidder’s annual turnover is N50m and above);
e. Evidence of Nigeria Social Insurance Trust Fund (NSITF) Clearance Certificate valid till 31st December 2024;
f. Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 31st December 2024; or valid certificate issued by BPP;
g. Sworn affidavit
– Disclosing whether or not any officer of the relevant committees of the University of Nigeria Teaching Hospital, Ituku/Ozalla, Enugu or the Bureau of Public Procurement is a former of present Director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bids are true and correct in all particulars;
– That no director has been convicted in any Country for any criminal offence relating to fraud or financial impropriety or criminal misrepresentation or falsification of facts relating to any matter;
– That the company is not in receivership, the subject of any form of insolvency of bankruptcy proceedings or the subject of any form of winding up petition or proceedings.
h. Company’s Audited Accounts for the last three (3) years 2021, 2022, 2023
i. Reference Letter from a reputable commercial bank in Nigeria, indicating willingness to provide credit facility for the execution of the project when needed;
j. Company’s Profile indicating its activities, with the Curriculum Vitae of Key Staff to be deployed for the project, including copies of their Academic/Professional qualifications such as COREN, QSRBN, ARCON, CORBON etc.:
k. Verifiable documentary evidence of at least (3) similar jobs executed in the last five (5) years including Letters of Awards, Valuation Certificates, Job Completion Certificates and Photographs of the projects;
l. For supply of equipment: Letter of Authorization as representative of the Original Equipment Manufacturer (OEMs):
m. Works: List of plant/Equipment with proof of Ownership/Lease agreement;
n. Electricity jobs: evidence of current license issued by Nigerian electricity Management services agency (NEMSA);
o. Consultancy Services (Lot 1S, 2S &3S): evidence of firm’s current registration with the Medical and Dental council of Nigeria (MDCN)
p. Consultancy services (Lot 4S): evidence of firm’s registration with relevant regulatory professional body e.g. ICAN, NIESV, ANAN, ESVARBON etc.:
q. ICT jobs: evidence of current license/accreditation with computer professional registration council of Nigeria (CPN) and National information technology development agency (NITDA)
r. For joint venture/partnership, memorandum of understanding should be provided (CAC, Tax Clearance Certificate, Pension clearance certificate, ITF compliance Certificate, NSITF Clearance certificate, IRR and sworn affidavit are compulsory for each JV partner)
Note: All documents for submission must be transmitted with a Covering/Forwarding letter under the Company/Firm’s Letter Head Paper bearing amongst others the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (preferably GSM No.) and e-mail address. The Letterhead Paper must bear the Names and Nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorized officer of the firm.

4. COLLECTION TENDER DOCUMENT (CATEGORY A AND B)
Interested bidders on presentation of an evidence of payment of a non-refundable tender fill of N10,000.00 per lot, paid into the university of Nigeria teaching hospital remita e-collection Account in a reputable commercial bank in Nigeria, shall collect standard bidding document during working hour at the office of the head, procurement department UNTH Utuka Ozilla Enugu.

NOTE: Consultant/Service Provider interested in Category (c) expression of Interest (EOI) are not required to pay for bidding documents until after short-listing.

ALL TERMS AND CONDITIONS IN THE COLLECTED BID DOCUMENT SHALL BE STRICTLY ADHERED TO PLEASE.

5. SUBMISSION OF TENDER DOCUMENT (CATEGORY A AND B)
Interested bidder are to submit their bids for each of the lot desired; Two (2NOS) hard copies (one original and one photocopy for each of the technical and financial bids,) package the separately in a sealed envelope clearly marked “technical bid” or “financial bid” also enclosed a soft copy of the financial.
Bids shall be prepared in MS-Excel format. Thereafter, put the two sealed envelop and the soft copy of the financial bids together in a bigger sealed envelope with the project name and lot number of interest clearly written on the top right-hand corner, addressed to:

Furthermore, the reverse side of each sealed envelopes shall be written the names and addresses of the bidder and dropped in the designated tender box at the office of the head, procurement department, not later than 12:00noon on Wednesday, 2nd October 2024.

6. SUBMISSION OF EOI
Interested consultants are to submit the expression of interest (EOI) for each of the lot desired, in three (3) hard copies each (one original & two copies). The EOI should be addressed as indicated above and submitted not later than 12:00noon Tuesday 3rd September 2024.

7. OPENING OF TECHNICAL BIDS
CATEGORY A & B INVITATION TO TENDER (WORKS AND GOODS)
Only the technical bids will be publicly opened immediately following the deadline for submission at 12:00noon on Wednesday, 2nd October 2024 at the UNTH Board Room, Ituku-Ozalla, Enugu in the presence of bidders or their representatives, while the financial bids shall be kept un-opened. Please ensure that you sign the bid submission register at the UNTH Board Room, Ituku-Ozalla, Enugu as the University of Nigeria Teaching Hospital, Ituku-Ozalla Enugu will not be held liable for misplaced or wrongly submitted bids.

CATEGORY C EXPRESSION OF INTEREST (EOI) CONSULTANCY SERVICES
Expression of interest (EOIs)n documents shall be opened immediately following the deadline for submission at 12:00noon Tuesday 3rd September 2024 at the UNTH Board Room, Ituku-Ozalla, Enugu in the presence of bidders or their representatives. Please ensure that you sign the bid submission register at the UNTH Board Room, Ituku-Ozalla, Enugu as the University of Nigeria Teaching Hospital, Ituku-Ozalla Enugu will not be held liable for misplaced or wrongly submitted bids.

CATEGORY C INVITATION TO TENDER (CONSULTANCY SERVICES)
The financial proposal shall be opened immediately following the deadline for submission at 12:00noon on Tuesday 3rd September 2024 at the UNTH Board Room, Ituku-Ozalla, Enugu. Please ensure that you sign the bid submission register at the UNTH Board Room, Ituku-Ozalla, Enugu as the University of Nigeria Teaching Hospital, Ituku-Ozalla Enugu will not be held liable for misplaced or wrongly submitted bids. 8. GENERAL INFORMATION
a. Bids/EOIs must be in English language and signed by an official authorized officer of the firm/company;
b. Bids/EOIs submitted after the deadline for submission would be returned un-opened;
c. Bidders should not bid for more than two (2) Lot;
d. All technical bids documents shall be accompanied by duly signed submission letters in company letterhead papers. Any unsigned bid(s) documents shall be disqualified;
e. The Technical Bids and EOI will be opened in the presence of Bidders or their representatives, CSOs in the areas of Anti-Corruption and Transparency and Private Professional bodies; and will be covered by video recording; invitation link will be sent to bidder who chooses to join the bid opening online;
f. Goods & Works: only pre-qualified bidders at technical evaluation will be invited at a later date for financial bids opening, while the financial bids of un-successful bidders will be returned un-opened;
g. Consultancy services: only shortlisted consultants will be invited at a later date for collection of requests for proposal
h. All cost shall be borne by the bidder;
i. The award for all projects listed above is subject to budgetary release from the treasury account under the FY2024 appropriation of the Federal Government of Nigeria;
j. The University of Nigeria Teaching Hospital, ituku-Ozalla Enugu is not bound to pre-qualify any bidder and reserves the right to annual the Procurement process at any time without incurring any liabilities in accordance with Section 28 of the Public Procurement Act, 2007.
